We maintain two optional waiting lists for individuals seeking to join our Family Health Organization (FHO):

1) General Waiting List

(no fee).

2) ChapelConnect List

(A membership program offering enhanced uninsured services such as digital tools, administrative support, and communication features.) 

 

Acceptance into the FHO is based on overall practice capacity, clinical considerations, and equitable access requirements.

Membership in ChapelConnect does not influence access to insured medical services or the timing of acceptance into the practice.
